At the final step, you have a few more options. The first option is to host the form on the web. To make your form public, click here. You’ll have the option of protecting your form with a Captcha, or requiring no authentication. You can copy the embed code here and put it on any webpage.

The next option lets you choose between a one- or two-column display.

Next, you can customize the name given to individual items that go through your app. This is called the subject. The subject will show in your reports section, and you can also use this when customizing your email messages. Use the open bracket to display a list of fields available.

In the last option, you can add other App Administrators who are able to edit this App and view the reports.
